Include the copyrighted work, specific allegedly infringing material, contact information, a good-faith statement, and a statement under penalty of perjury that you are authorized to act.
Counter-notice
A counter-notice should identify removed material, state under penalty of perjury that removal was a mistake, provide contact details, consent to the appropriate jurisdiction, and include a signature. Send counter-notices to the same email or postal address.
